Registered Nurse, RN

The purpose of this role is to improve patient outcomes, supporting treatment in a timely, safe manner and developing, deploying and coordinating the patient’s Care Plan in line with their needs

Responsibilities

  • To deliver the service to enrolled patients both face to face, through homecare visits,  and via telephone and virtual calls, in line with the patient blueprint and as required with the patient.
  • Assist in the coordination of the prescription process. Working closely with pharmacies to minimize delays in patients receiving treatment. 


  • To deliver, or administer or train patients on any diagnostic tests that may be required.
  • Responsible for developing, agreeing and coordinating the patients individual Care Plan. To include the ongoing assessment, product and disease education, implementation and evaluation of the patient needs and capturing efficacy measures.

  • Provide advice and education on treatment, plus disease management and lifestyle recommendations. 
  • Record all patient data using the CRM system and ensure all administration is completed in a timely and accurate manner. 
  • Working professionally and effectively to build relationships with all stakeholders

Essential Requirements

  • Current U.S.  driving licence  and an ability and willingness to travel to  patients’ homes within an acceptable geography.
  • Excellent interpersonal skills and communication skills.
  • High levels of confidence, initiative & proactivity.


Licensed Practical Nurse, LPN

Provide one-on-one skilled nursing care and treatment to patients in their home. Use a comprehensive and patient-specific care plan . Spend quality time with the patient, in their own home, to promote their health and independence. Collaborate with physicians, patients, families, nurses, therapists, social workers and other members of the patient care team to implement and follow a patient specific care and treatment plan. Educate and promote the health and independence of the patient.

Education/Training:

Graduate of an accredited school of practical nursing.

High School Diploma


Licensure/Certification:

Valid Licensed Practical Nurse in the State of MA
CPR certification /BLS


Certified Nursing Assistant, CNA

Key of care nursing is hiring for Per Diem, Part Time and Full Time CNAs

Position Summary

The Certified Nursing Aide is responsible for providing routine daily care to assigned patients and  assure patient safety and attain or maintain the highest practicable physical, mental, and psychosocial well-being of each patient. iReports to nurse Case manager.

Education, Experience, and Licensure/Certifications

  • Must be a graduate of a State-approved CNA program.
  • Must be currently registered/licensed in applicable State. Must maintain an active license in good standing throughout employment.
  • Must have CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment.
  • High school Diploma or equivalent


Essential Functions

  • Must be able to lift, turn, move, position, and transport patients.
  • Must be able to assist patients with baths, showers, personal grooming, feeding, and toileting.
  • Must be able to accurately document and chart patient care.
  • Must exhibit excellent customer service and a positive attitude towards patients.
  • Must demonstrate dependable, regular attendance.
  • Must be able to communicate and function productively on an interdisciplinary team.
